Patient Details: Upendra/Male/45 Yrs

PHC Name: Koparli, Vapi, Valsad, Gujarat

Presenting Complaint: Skin issues

Consultation/Follow-up Date: February 14, 2025

Upendra, a 45-year-old farmer from Koparli village, recently visited the Phygital PHC Telemedicine Centre for a follow-up consultation regarding his skin condition. Previously, he struggled to access dermatological care due to the high cost and long travel times to the nearest hospital. Since his initial consultation at the Phygital Centre, his chronic skin rash and itching have significantly improved with prescribed medications and professional guidance.

During his follow-up, the dermatologist assessed his progress, adjusted his treatment plan, and provided further skincare recommendations. Upendra expressed satisfaction with the convenience of the service, as it eliminated the need for costly trips to the city and provided timely medical attention.

Upendra thanked Dungra Phygital PHC staff Trupti for her care and support. He now actively encourages others in his village to use the Phygital Centre, recognizing its vital role in making specialized healthcare accessible in rural areas.
